Over the last week, we received a number of inquiries regarding our registration process. The registration form is a College of Physicians and Surgeons of BC requirement in their recent guidelines, to capture a snapshot of you as a patient. As physicians, we need to know what medications you are taking and what medical conditions you have to ensure that we do not provide you with treatments that could potentially harm you.
We decided to launch an electronic version of the form to allow patients to register at home, work or simply when they sit in our waiting room using their smartphone or iPad. We will still require your healthcare card to verify who you are, but now we are able to enter you in our system ahead of time, to ensure that your experience, when you are ill, is pleasant at all times.
For those that feel more comfortable with paper, we will require the form to be completed at our clinic before you are seen, to ensure that the physician treating you is completely informed at the time of seeing you.
We added two examples to this post for you to look at.
